If you are at present employed as a full-time or part-time teacher in a school or educational establishment, please complete this section
PLEASE NOTE: If you have completed this section, we would prefer that a colleague on staff of the same school or college would act as your professional referee. However, if for personal reasons this is not appropriate, we will accept another professional referee.

DBS Certificate
If you do not have an Enhanced DBS Certificate, this can be arranged for you through an authorised checking company. The current cost is £59.60.
If the answer is YES to either of these questions, please contact us and let us know in confidence. Dependent on its nature, a criminal record doesn’t necessarily prevent your registration as a tutor with us. Any police record does show up on a DBS check but we know that often some of these are long in the past and would not prevent you being a tutor with us. We do however need to know, in order to comply with Child Protection procedures and with the Rehabilitation of Offenders Act 1974 (exempt) Amendment Order 1986.

Tutoring Agreement - you must read this and confirm you have done so below
THE AGREEMENT

a) I have completed this form honestly and will email the documentation requested.
b) I would like to be recommended by HST as a self-employed Tutor to provide professional tutoring services.
I agree with the Home-School Tutoring Tutor Responsibilities and your Terms and Conditions as sent to me.
c) I understand that you negotiate the best and appropriate Tutoring Fees for me with the Client.
d) I understand that the payment arrangements for my tutoring, may vary.
e) In some circumstances, HST will invoice the clients and I will receive my tutoring fees in arrears.
f) In other situations, payments will be made immediately by the Client and I collect those payments.
g) The payments passed to me will have been agreed by Home-School Tutoring and they will include my agreed tutoring fees and the client HST Admin Fees.
h) I then forward to you those HST Client Admin Fees at the end of each month (you will confirm those to me). I accept that I will incur interest charges if late in forwarding those Admin Fees.
i) The HST Admin Fee per hour, as quoted to me when taking on pupils with Home-School Tutoring, is due from all clients whom I have been introduced to by Home-School Tutoring, whether directly from HST or indirectly, for as long as I tutor these pupils. This includes siblings, relatives or friends of a previous or current HST introduction and clients who return for tutoring after a break or for another subject or level. HST will inform me of the Admin Fee for each assignment, whether 20% (generally) or 10% for direct tutor-approach pupils as a result of the original HST referral.
j) I understand that my referees, who will vouch for my personal and professional character, can be contacted now or if I should default or cause concern whilst in a position of trust, according to this declaration.
k) I understand that neither this Area of HST nor HST UK will be held responsible for any injury, loss accidental damage, personal indictment or any other claim whatsoever arising in connection with my tutoring or teaching assignments.
l) I understand that the information contained on this form will be held either on computer or otherwise (please see our privacy policy below.)
m) (For child protection purposes and for best professional practices, I confirm that I will read and adhere to all HST policies, including Safeguarding and Health and Safety, and to the Code of Ethics. I will either be sent these policies by my Area Advisor by email attachment or, once I am fully registered with HST, I will find these policies in the Tutor Resources sign in section of the website www.homeschooltutoring.co.uk. (A password will be available for me to access this section once I am fully registered) please request this password from your Area Advisor or from HST UK e: office@homeschooltutoring.co.uk.

We confirm that once you are registered with us, we store your personal information in accordance with governmental privacy legislation.
Your registration with us means that you are willing for us to give out to potential Clients your contact details and other relevant information, including your qualification and experience details, where tutoring work has been assigned to you.
While you are registered with us, we will use your email or contact details to contact you from time to time.
If you wish to de-register with us with us as a tutor, please let us know by email. We will de-register you and we delete your contact details if you choose to no longer hear from us nor receive any newsletter or further offers from us.
Your personal details may be disclosed to any person, agency or government department if we are required to do so by law.
